Questionnaire
We're excited to get started! Before we can schedule a consultation we need to contact your insurance to check your plans coverage. That way we can return your call with your benefits, and answer any questions that you may have. Please complete all fields in this form to get started.

Our Process
Whether you're ready to start scheduling sessions or your inquiring about covered services, a few things need to take place before we can begin.
Step One
Complete the Questionnaire
Once received, we will call your insurance to check your benefits, eligibility, and see if a pre-certification is required (almost always is). At this point if you decide your ready to start services for your child, we go over the billing process and send out intake forms. Once we receive the filled out forms and insurance approval we can schedule the interview.
Step Two
In-Person Interview
A Board Certified Behavior Analyst (BCBA) will be scheduled to come to the home and conduct an interview the primary caregivers and review school records and medical documentation. Here we learn the child's general background history, identify the parents concerns, and choose an appropriate psychological assessment to use next visit.
Step Three
Assessment
A Board Certified Behavior Analyst (BCBA) will be scheduled to come to the home and conduct an initial assessment with your child. The results serve as a baseline before treatment begins as well as identifies the areas to target for development. They are also a great tool to use to re-evaluate your childs progress in the future.
Step Four
Insurance Approval
We submit the assessment with a request for treatment hours to your health insurance for approval. They typically process requests within 2 weeks. Once we receive the approval we will reach out to schedule.
Step Five
Scheduling
We contact you to review the hours authorized and set up dates and times for therapy sessions. Session duration and frequency will depend on your availability and childs needs.
